Hammersmith & Fulham Mind manages 5 shared homes for adults with enduring long-term mental health issues who require a medium level of support. Some residents may have more complex needs. Supported Housing staff work with residents on an individual basis both in their homes and also within the community.
Threshold and Metropolitan Housing are the Registered Social Landlord and work in partnership with Mind as the managing agents and support provider for two of the houses. Notting Hill Housing Group is the managing agent for three houses and works in partnership with Mind who are the support providers.
The properties are scattered in the Fulham, Hammersmith & Shepherds Bush area.
Residents have assured shorthold tenancies and are provided with support in managing their tenancy in preparation for move on to more suitable accommodation when they have developed the skills and confidence to do so. The expected maximum stay is usually for no longer than two years.
We operate a whole team approach with the main aim being to enable people to be as independent as possible within their chosen lifestyle and regain confidence in maintaining a tenancy. This is achieved through regular support planning and assessment of risk. We develop positive therapeutic relationships with each resident using the client-centred approach. One of our main aims is to ensure that the resident has the opportunity to utilise the other services that Mind provides and assist the resident in areas such as building up support networks and activities whilst appreciating each person as an individual and respecting their personal choices. We actively encourage resident participation particularly in how we manage the housing services and value the input from the residents. In addition, support our residents in becoming involved in the wider community.
When a vacancy arises, the Joint Commissioning Officer for Mental Health Accommodation is notified. Referrals for the supported housing service are made through the support panel.
